Advanced Care Strategies is a service provider

for primary care offices, home health agencies, assisted living and long-term care facilities as well as CILA homes.

 

We hire LSW's, RN's, LPN's, CNA's, DSP's, QIDP's, social service directors, activity directors, nurse trainers, floor nurses, restorative nurses, wound nurses, DON's, transitional health managers. 

We have trained geriatric care managers and can take long-term continuous clients or we can act as LTC referral agent where the individual is not charged a fee.
